Disclosure of medical records

I understand that Health Consultants Inc will use and disclose my health information for the purposes of treatment, payment and healthcare operations, as permitted by law under the data protections act 1998.

• I understand that Health Consultants Inc may record medical and other information related to my treatment in paper, electronic, photographic, video and other formats and that such information will be used in the course of my treatment, for payment purposes and to support healthcare operations.
• When responding to medical file requests we are required by law to acertain in writing that the party making the request has your permission to acquire the requested information.
• There is an administration charge associated with the delivery of medical reports for third parties. We aim to respond to all medical requests within 28 days from receipt of payment for the records.
